Billing & Insurance

Health insurance is filed as a courtesy by our staff. We ask that patients please provide current and accurate information regarding insurance coverage. We are participating providers for Medicare, Medicaid and various HMOs and PPOs. All co-payments are expected at the time of service. Any outstanding balance that remains after payment from the insurance company is due within 30 days. We accept cash and checks. View our list of accepted insurance plans.

I am a new patient. Which forms should I complete?

The forms that need to be completed are General Consent for Treatment, Notice of Privacy Practice Acknowledgement, and Release of Medical Information. Please visit our forms page to download and print all forms.

How do I obtain test results?

You will be notified at the time of the test as to how long it should take for the results to come back. Typically test results are within 24 - 48 hours, however, some results may require more time. If you have not heard anything within 72 hours, please feel free to call the office.

What should I do in case of emergency?

You may always call the office in the event of an emergency. During normal business hours, a staff member will answer and assist you. After hours, the answering service will page the physician who is on call. If you feel that the emergency is life threatening, please call 911.

What should I bring to my appointment?

Please bring all of your medications or a complete list (including strength and dosage) of current medications with you to your visit.

How do I request a prescription refill?

We would ask that you request all refills at the time of your visit, however, if you need to request a refill between visits, please call the main number to the office where you are seen and your call will be directed to the appropriate person.

How can I reach my doctor's nurse?

You may speak to your doctor's nurse or medical assistant by calling the main office number at 919-784-7550. The staff member who answers the phone will route your call appropriately.
